2019 Guatemala Trip Info
If you have been on a trip with Caroline's Promise in the past 3 years you need to complete the online application and the following download forms:
* Short Term Mission Agreement
* Code of Conduct
* Adult Waiver
* Insurance & Medical Release
* Minor Health Release if you are a Minor
If you are a first time applicant you will need to complete and download all forms.
Everyone needs to upload a copy of their passport. If you have not applied for your passport you will have an opportunity to submit it once it arrives.
Application Deadline: February 25, 2019
A $100 non-refundable deposit is due at the time of application. This will be applied toward your total trip cost. Please donate this $100 deposit at this link. Choose "Guatemala Mission Team General" and write your name in the field, "Whose team are you giving to?"
